IMPART’s Certification Programs for Psychics, Mediums, and Healers

Application Form


NOTE: Sadly, IMPART’s CERTIFICATION PROGRAMS were discontinued as of July 2020 when Anne had to close her Center due to the COVID-19 pandemic.


After completing the minimum entrance requirements for IMPART’s Certification Programs, please complete the Application Form provided below.

  • Your completed Application Form may be submitted using any of the following methods:
    • Hand-deliver to Anne
    • Scan and email to Anne at
    • Mail to the Impart Wisdom and Wellness Center, 540 N. Golden Circle Drive, Suite #108, Santa Ana, California, 92705
  • Your application will not be processed unless you select at least 1 Certification Program (i.e., Psychic, Medium, Healer).
  • Your application will not be processed until your Registration Fee is received.  Registration Fees are currently $150 for 1 Certification Program, $250 for 2 Certification Programs, and $300 for 3 Certification Programs.  You may pay your Registration Fee using any of the following methods:
    • Hand-deliver to Anne (cash, check, money order)
    • Credit card payment (e.g., contact us to make arrangement at (714) 599-0017 or

Once your Registration Fee has been received, your Application Form will be reviewed and you will be informed regarding the status of your acceptance into the Certification Program(s) of your choice.  If you are not admitted, then your Registration Fee will be returned to you in full.
